Question #1:

Do you have a mobility limitation that significantly limits your ability to participate in one or more activities of daily living such as feeding, dressing, grooming and bathing safely and in a timely period?

Question #2:

Are you unable to use a cane, walker, manual wheelchair, or scooter to safely complete your in-home activities?

Question #3:

Are you able to safely transfer to and from a power wheelchair, operate the joystick and sit upright while operating a power wheelchair?

Question #4:

Are you able to safely operate a power wheelchair inside your home?
