Insurance Billing at Spinlife

Thank you for shopping at SpinLife. We are happy to assist you in obtaining reimbursement from your insurance company for any eligible item(s). Eligible items will be marked on the website with the following symbol: Purple Insurance Icon.

SpinLife files reimbursement claims to insurance companies as a courtesy service only; we do not take assignment and cannot be billed by the insurance company directly. We are happy to assist you in verifying your out-of-network benefits on a specific item before you make your purchase. However, the item will need to be paid for in-full before we can assist you in filing for possible reimbursement. If pre-authorization is required by your insurance company, SpinLife.com will forward the medical documentation you provide to us onto your insurance company for their review.

Please note: SpinLife is only able to submit reimbursement claims for customers who do not have an HMO as their primary coverage. Also, remember that coverage terms are different for each insurance company, so we recommend that you check with your insurance company to determine whether or not an item is eligible for reimbursement. SpinLife will always be an "out of network" provider. However, SpinLife prices are so much lower than the "in network" providers that it is generally less expensive for you to work with SpinLife.

What to Expect

When you place an order or create a quote (either online or by phone) and indicate you would like SpinLife to bill your insurance company for reimbursement, you will be asked to provide your health insurance information including your subscriber #, group #, date of birth and your insurance carrier's member services phone number.

After submitting your order, you will receive an information packet via email (or by regular mail if you do not supply a valid email address). This packet includes detailed instructions about how to provide the information required to complete insurance reimbursement billing.

The process involves these simple steps:

  1. Contact your insurance company to verify that the item you are interested in is eligible for coverage.
  2. Place an order or create a quote with SpinLife (either online or by phone) and indicate that you would like SpinLife to bill your insurance carrier for reimbursement. (Note: As long as your doctor's prescription is dated before the delivery date of your item(s), you do NOT have to wait to complete and pay for your order!)
  3. Provide your health insurance information including your subscriber #, group #, date of birth and your insurance carrier's member services phone number as required.
  4. SpinLife will contact your insurance carrier to confirm eligibility and specific coverage. Our billing specialists will then contact you with the details, including your reimbursement eligibility.
  5. SpinLife will send you an information packet via email (or by regular mail if you do not supply a valid email address). Please carefully review the packet and follow the instructions to have the paperwork completed and returned to SpinLife.  
  6. Collect the following information:
    • HIPAA Release Form: SpinLife abides by all required confidentiality regulations. In order for your physician to release relevant medical information to SpinLife, we must have a signed HIPAA form on file.
    • Prescription: The prescription must be dated before the delivery date of your order to be valid. The prescription must also clearly state the item that is needed, the lifetime need of that item and any diagosis codes
    • Letter of Medical Necessity: This must be on the physician's letterhead to be valid.
    • Medical Information: Please include copes of any recent evaluations, physician's notes, diagnostic tests or lab work, etc.
  7. Send the required information (above) in one of the following ways:

  8. If you have not already done so, you will pay in full at this time and SpinLife will process and complete your order.
  9. SpinLife will file your reimburesement claim with your insurance carrier within seven days. Your insurance carrier will process your claim and generally reimburse you within 30 days.
