Coverage terms are different for each insurance company, and SpinLife recommends that you check with your insurance company to see if the item you need will be covered. SpinLife will always be an "out of network" provider. However, SpinLife prices are so much lower than the "in network" providers that it is generally less expensive for you to work with SpinLife.
In many cases, SpinLife can assist you in obtaining a pre-authorization from your insurance company so you know your level of reimbursement before you pay for you order. Some insurance carriers will actually guarantee payment directly to SpinLife, eliminating any up-front payments from you, except possibly your co-pay.
SpinLife is only able to process claims for customers who do not have an HMO as their primary coverage. SpinLife will verify your coverage and eligibility before processing your order.
What to expect
When you place on order or create a quote (either on the phone or online), and indicate you would like SpinLife to bill your insurance company, you will be asked to provide your health insurance information including your subscriber #, group #, date of birth, and member services phone number. (You will be asked to indicate insurance billing during the checkout process). You will then receive an information packet via email (or regular mail if you do not have an email address) that includes detailed instructions regarding the necessary information required and the options for sending the information to SpinLife.
The process involves these simple steps:
- You begin by contacting your insurance company to verify that the item is eligible for coverage.
- Place on order or create a quote with SpinLife (either on the phone or online), and indicate you would like SpinLife to bill your insurance carrier. You will be asked to provide your health insurance information including your subscriber #, group #, date of birth, and member services phone number.
- SpinLife will contact your insurance carrier to confirm eligibility and specific coverage. Our billing specialists will then contact you with the details, including your total out-of-pocket cost after reimbursement.
- SpinLife will send you an information packet via email or regular email. Please carefully review the packet and follow the instructions to have the paperwork completed and returned to SpinLife. As long as the necessary prescription is dated before the date of delivery of the item, you do NOT have to wait to pay for and complete your order.
- Send the following information to SpinLife:
· Copy of the Prescription Stating Item Needed, Lifetime Need, Diagnosis Codes
· Copy of Letter of Medical Necessity on Physician Letterhead
· Copy of any Recent Evaluations, Daily Notes, Diagnostic Tests/Labs (MRI, X-ray, etc…)
· HIPAA Release Form. This gives your physician permission to release relevant medical information to SpinLife so that we can successfully submit your claim.

6. If you have not already done so at the time of the order, you will pay for your order in full and SpinLife will process you order.
7. SpinLife will file your claim with your insurance carrier within 7 days. Your insurance carrier will process your claim and generally reimburse you within 30 days.
